| Description of effective Public Awareness and Child Find activities implemented in rural areas of your service region.

- Advertised in a local paper twice a month for a year. Also ran the advertisment in Spanish. We also have public service announcements on the radio in English and Spanish.
- We have a booth at the Family Fair which covers 5 counties.

| Collaboration with other agencies in implementing public awareness or child find activities.

- We purchased a van in conjunction with Success by Six. The van has the logos and telephone numbers of both agencies on it. Public health nurses operate the van and drive it around the community providing developmental information, assessing children and then make referrals if appropriate. Parents can call to have the van and personnel come to their home. Ads are run on TV providing information about the van and the number to call to ask about it. They automatically go to homes of newborns and provide parents with developmental information and a number to call in the future if they have any concerns. This program started October 1999, but they did not get the van into the community until May 2000, so the program is very new. But it appears at this time that it has been successful in reaching the more rural populations.

| Process used to plan and implement public awareness and child find activities.

- Family Resource Coordinators log their activities and turn them in once a month and they also have weekly meetings.
- We have a volunteer coordinator for Public Awareness and Child Find activities who meets weekly with a supervisor.
- Each family resource coordinator is also responsible to visit physicians in their city and provide them with a poster and prescription pads.
