“Creating Transition Portfolios for Middle School Students with Disabilities”

This free teleconference training event will provide you with the “nuts and bolts” to create a Transition Portfolio for a middle school student with a disability. The Portfolio includes pictures and text to capture a young person’s interests, connections to the community, work history (including chores), skills and abilities. Portfolios can be used to share information about students’ preferences, strengths, accommodation strategies and support needs at Individualized Education Plan meetings and are great tools for enabling students to communicate about themselves at their IEPs. Information from the Portfolio can then help guide curricular content, as well as volunteer and employment experiences. They can also be used as a way of introducing the student to new teaching staff and provide a tool families can carry with them as their young adult navigates his or her way from middle school to high school and beyond.
