Contact Your Pass
Cadre When...

Contact the PASS Cadre Immediately If:

• Your vocational goal changes;
• You can not pursue your vocational goal temporarily or permanently (due to health or other factors);
• You need approval to change your budget or selected vendors;
• The amount that you can put into your PASS account changes;
• Your representative payee changes or your address changes;
• Your employment status changes; or
• You need more time to complete your plan.

PASS Plan Amendment:

• If your vocational goal or PASS plan changes slightly you can request the PASS Cadre to amend your plan:
• Simply edit your original PASS plan, which ideally you have saved on your computer;
• Type PASS PLAN AMENDMENT at the top of the form and highlight it;
• Type all changed information on the form and highlight all the changes; and
• Enclose a cover letter with the amendment explaining why the changes are necessary.

PASS Plan Extension:

• If you need more time to complete your plan:
• Edit your original PASS;
• Type PASS PLAN EXTENSION on the top of the form and highlight it;
• Type all changes and highlight the changed information; and
• Include a cover letter explaining why more time is needed.
